Admission Requirement

 

Enrollment in our program is open to all families in our community.  We operate on a non-discriminatory basis.  No one shall be excluded because of race, color, religion, disability, sex or national origin.  All forms provided at enrollment must be completed before your child can attend Dameron Daycare.  All information is kept confidential.  Parents are required to update emergency information as needed.  Parents are requested to comply with all center rules as set forth in this Parent’s Handbook.

 

Forms Required for Enrollment

 

•          Physical Form signed by the Parents/Guardian and Physician

•          Proof of Lead Screening signed by a Physician

•          Immunization Record signed by a Physician

•          Emergency Card containing child’s name, address, telephone number, date of birth, date of enrollment, list of people authorized to pick up the child in the event of an emergency and the parent cannot be reached.

•          All About Me form

•          Signed form stating you received a Consumer Education Pamphlet

•          Signed Contract detailing the hours care is to be administered, the type of care to be administered, the payment amount, and the payment frequency.

 

Infants entering into care must also submit:

 

•          A schedule for feeding that includes the amounts of food and the type of food the child is to consume

•          The sequence for introducing solid food when appropriate

•          And recommendations about feeding from the infants physician

•          A written individual activity plan
